New Change of Major Form for students with more than 90 units

The University has a new change of major/minor form for students who have earned more than 90 units, effective immediately.  The form requires the same information, plus a personal statement that explains how the change of major/minor will impact graduation plans.   To download the new form, click here.

Stop by on Wednesday this week and get your questions answered 

and your plans moving before the term begins !!!

Some of you are newly admitted majors, some will need to submit graduation paperwork early this term, others have questions about general education (R,S,&V) requirements, and still others have questions about class sequencing.   All Health Science Students are going to be required to create a long-term academic plan this spring, so, don't wait. Let's get started on your planning now.

New Change of Major Students - Welcome to HS!

Congratulations to all of our new Health Science Students who were recently admitted through the change of major application process.   Here is a brief overview to advising options and how to submit required forms to formally declare HS as your major.

Spring 2015 Change of Major Application 12/29/14 - 1/6/15

The Health Science Undergraduate Program will be accepting new students through a change of major application process for Spring 2015.  Our online application will be available on Monday, December 29th, following the release of Fall grades, and the application period will close on Tuesday, January 6th at midnight.    

You will need to access you unofficial transcripts that include Fall 2014 coursework in the GPA calculation and you must have successfully earned 12 units at SJSU to be eligible to apply.  All applicants will be ranked by GPA - either SJSU or all College Cum, whichever favors the applicant - and we will accept students starting at 4.0 until we have filled our openings.    

The application link will be sent to everyone who has indicated interest in the program through the HS Office and it will be available here - on the HS Advising Blog - as well. 

Make sure you DO your course evaluations

Course evaluations are online through my.sjsu and you have until tomorrow (Wednesday 12/10) to complete these for all of your fall courses.    The evaluations are really helpful for your instructors and they give everyone an opportunity to reflect on what is working well in a course and what might need to be changed in the future.  Please take the time to share meaningful and productive feedback so that faculty and departments can provide students with consistent high quality educational experiences.   Evaluation results are completely anonymous, so please provide an honest reflection of your experience in the course.    Once you are in the system, you will have an option to opt out of the survey if you choose not to evaluate a course or instructor, but I encourage you to offer important feedback.

If you do not complete all of your evaluations (or opt out of them) you will not be able to access your grades in your current fall courses until well into January of 2015.  This is particularly important for students who are wanting to apply to Health Science in the change of major process because we need your fall grades included in this application.
