Graduating in Fall 2016 or Spring 2017?

If you are starting to think about graduation and you're ready to apply - please come to one of the advising workshops.  You can drop in anytime during the workshop and a faculty advisor will be available to assist you with your process, answer questions about the forms, and help you get your paperwork completed.

For Fall 2016 Graduates, your forms are due to the Health Science Office before March 15, 2016 so we can get them approved, processed and back to you in time to meet the University deadline (firm) of April 1, 2016.

For Spring 2017 Graduates, your forms are due to the Health Science Office before April 15, 2016 so you can meet the University (firm) deadline of May 1, 2016 for two semesters of priority registration.

The forms that you need to complete include the University Graduation Application form and the Health Science Major Form for your declared major in our program:  Health Science (with electives or a minor) or Health Science with a concentration in Health Services Administration.
