Graduation Process

All students must apply for graduation by submitting the University Graduation Application.

You will submit two completed forms to the Registrar:

• University Graduation Application Form 
• New online form being piloted in our college for Spring 2017 and Summer 2017 graduates - online application due September 15, 2016 - available from CASA at: http://www.sjsu.edu/casa/graduation/. Students who miss the online application filing deadline can still submit a paper form by the October 1st deadline.
• Students who are eligible to file the graduation application electronically will be sent an email on Monday, August 15th with more information.  
• Health Science Major Form (BS, HS with a minor or electives or BS, HS Health Services Administration Concentration) 
• Still required to be submitted in paper by University deadline
• Declared Health Science Minors will need to submit a HS Minor Form

Completed forms need be approved first by a Health Science Faculty Advisor and then the registrar.  You can submit the forms up to one year in advance of your graduation date and you should begin this process at least one month prior to the University deadline.  For more information on the graduation application process, including deadlines, please review the information provided by the office of the Registrar.

Once your graduation application has been received and processed by the registrar's office you will receive a Graduation Worksheet.  This worksheet will be sent via email from your graduation evaluator and you should review it.  For more information on understanding this worksheet, please see the Registrar's Graduation Worksheet Guide.
